In order for us to better help your client we ask that you include the following information in your referrals: Your agency's name, and contact information (preferably your letterhead) Contact Information of Individual referring the client Name -Full Name of the referred client Number In Household -How many are currently living in the household Specifics On Household Individuals- -Name -Age -Gender -Relation to Client -Income Specific Need(s)- what does the client specifically need; Please be as detailed as you can.
